LLC 2023 |07.23 HCBS WAIVER SERVICES COMMUNITY OPTIONS Residential Habilitation A variety of supports based on your wants, needs and goals, provided in OPWDD Certified Residences. Community Habilitation A variety of supports to meet your needs &goals, promoting independence &community integration. Day Habilitation Habilitation services provided outside of the home with options to fit your wants, needs, and goals. Pathway to Employment Time-limited career planning &support service. Assists with obtaining, maintaining or advancement in competitive employment. Often leads to Supported Employment. Pre-Vocational Services Provides learning and work experience, including volunteering, to develop skills related to employment. This can be both site and community based. Supported Employment (SEMP) Ongoing support to obtain and maintain employment. Respite Temporary caregiver relief for people unable to care for themselves. Not a habilitation service. Assistive Technology/Adaptive Devices An item, piece of equipment or product system that is essential to habilitation, ability to function, or safety. Environmental Modifications (Home Accessibility) Physical adaptations made to the home, necessary to ensure health and safety, while allowing for greater independence within the home. Vehicle Modifications Physical adaptations to your primary vehicle. Family Education/Training Focused training to support your family on a variety of topics. HCBS Waiver Services are offered through OPWDD and funded by Medicaid. You must be enrolled in Medicaid and the HCBS Waiver to apply for these services through OPWDD.
