Once you are eligible for Disability Supports and Services through OPWDD, and have enrolled in Medicaid
and the HCBS Waiver, Enrollment Specialists can help guide you through enrollment in your CCO of
choice for access to Care Management services.
WHAT IS THE HOME AND COMMUNITY BASED SERVICES WAIVER (HCBS)?
The HCBS Waiver is operated by OPWDD, the Office for People with Developmental Disabilities.
It provides supports and services for people with disabilities in the community and at home that are
uniquely tailored to meet the needs and wishes of the person.
These services are funded by Medicaid. Medicaid approval can take several months.
To receive these services, you need to:
Be eligible for OPWDD
Have Medicaid
Enroll in the HCBS Waiver program
CARE COORDINATION SERVICES ENROLLMENT
Once eligibility and enrollment with OPWDD and Medicaid are complete, it’s time to enroll in a Care
Coordination Organization (CCO).
CCO enrollment is effective the first of the following month after OPWDD eligibility and Medicaid is active.
Medicaid is the only insurance accepted for Care Management services through a CCO.
Once enrolled, your CCO Care Manager will work with you and your support system to develop a detailed
plan of care, called a Life Plan.
The Life Plan process includes identifying the need for Home and Community Based Waiver Services,
administered through OPWDD.
DO I EVER HAVE TO RE-APPLY FOR SERVICES?
OPWDD Provisional Eligibility Services are granted to a person under the age of 8.
If the person still meets criteria after the age of 8, then eligibility is re-established.
If Care Management services are terminated, you must re-enroll in the CCO in order to begin receiving
these services again.
