The Life Plan is a document required for anyone receiving OPWDD services. The Life Plan guides the

THE LIFE PLAN
• The Life Plan is created and subsequently updated in a process that involves you, your Care
Manager, anyone you wish from your Circle of Support, and usually, but not always, your service
provider agencies.
• Your Life Plan should reflect your goals, dreams and preferences.
• The process focuses on health and behavioral health services, community and social supports,
and more.
• The Life Plan process assists in accessing services.
• This process occurs at least every 6 months, but can be adjusted as your goals or needs change.
