DRAFT Pre-Meeting Life Plan Review
• The /ife Plan is reviewed and updated by the Care
Manager.
• The Life Plan is sent to you and anyone you wish fRU your
Circle of Support.
Life Plan Meeting (annual, semi-annual)
• You, your Care Manager, and desired Circle of Support members, meet
to review the Life Plan.
• Updates and changes to existing Life Plan are discussed.
After the Life Plan Review Meeting
• Care Manager updates the Life Plan and sends to your team for feedback.
• Updated Life Plan is sent to you for approval.
• You or your representative signs the Life Plan if approved.
FINAL Life Plan Finalized
• The Life Plan is finalized after you or your representative have provided
your Care Manager with a written signature
• The Care Manager will also apply their digital signature to the plan and
send it to waiver providers (when required) for acknowledgments and
agreement.
• If no waiver providers, the Life Plan is considered complete, and you
and your team receive the finalized plan.
ACKNOWLEDGED
AND AGREED
Acknowledged and Agreed
• Once fully acknowledged and agreed by you and waiver providers,
the Life Plan is complete and is distributed to you and your team.
